What if I get a message that my identity could not be verified?

You may want to try a previous address. If that still doesn't work, you can click Save and Continue 4 times. This will allow you to request the certificate online but you will need to email in proof of identity once your order is complete. You will email that to vrequest@utah.gov. Make sure you include your order number.
