Licensee Verification Information

An official electronic Licensee Verification will be emailed from the Department of Professional Licensing directly to anyone you choose, such as directly to you, a government agency in another state, or to a potential employer.

Note: You must verify the government agency in another State will accept this email as an official verification.

To send a Verification for License #

1. Determine Delivery Instructions
2. Pay $5.00 per email address through a credit card or by a Utah.gov Subscriber Account
3. DOPL will directly email a PDF document to each email address selected
