Based on your registration, you will need to print the following file, sign and deliver it to your county clerk. You may mail or walk in the information. See deadlines below.

Your registration was not able to be automatically submitted for one of the following reasons:
1) Your signature was not stored digitally at the Driver License Division
2) You modified your address
3) You selected that you did not want to use your Driver License Signature
4) You modified your address or date of birth so that it does not match the information on file at the Driver License Division

The form must be postmarked at least 30 days before an election to be eligible to vote in that election.

You may also walk this form into the County Clerk's office at least 7 days before an election to be eligible to vote in that election.
